Gives you a pretty simple barcode, start with a white background

Add gaussian noise (monochromatic) at 400%

Apply the motion blur filter at 90 degrees, distance 999 pixels

Now mess around with the curves to brighten it up

Select the area you want for the size of your barcode, then invert and cut

Thats it, just cut a section out of the bottom and add some text
